Objective:To assess the effect of sildenafil (Viagra®) on the cardiovascular system and on the cardiovascular risk profile in patients with coronary heart disease. We included 1090 patients with coronary heart disease treated with sildenafil (Viagra®) and 990 patients with chronic stable angina.
Design:The study was a case-control design. Patients were recruited from two centres in Hong Kong: King Abdulaziz General Hospital (KGH) and Chiang Mai University College of Medicine (CMUMC). The patients were treated at KGH and CMUMC from 2012 to 2012.
Setting:The study was carried out at the CHUMC, and the patients were included in the follow-up period between 2012 and 2012.
Patient characteristics:The baseline characteristics of the 990 patients with coronary heart disease are shown in Table 1.
Treatment type:The sildenafil group was treated with sildenafil at the highest effective dose (5 mg/day). Patients who were prescribed sildenafil to reduce cardiovascular risk factors had higher rates of CHD death, CHD hospitalization, and higher number of CHD events than the other treatment groups. There were no significant differences in the rate of CHD death, CHD hospitalization, or use of other types of antihypertensive drugs among the patients treated with sildenafil and in the patients treated with the lowest effective dose (50 mg/day). There were no significant differences in the rate of CHD death, CHD hospitalization, use of other types of antihypertensive drugs, and use of any type of antihypertensive drug between the patients treated with sildenafil and those treated with the highest effective dose (5 mg/day). The rate of CHD hospitalization was similar among the sildenafil and the other treatment groups.
Primary outcome:The primary outcome was the cardiovascular event rate. The rates of fatal, nonfatal, and nonfatal myocardial infarctions were similar between the patients who received sildenafil and the other treatment groups.
Secondary outcomes:The primary outcome was the cardiovascular event rate for the patients who received sildenafil and those who received sildenafil at the highest effective dose (5 mg/day). There were no significant differences in the rates of fatal, nonfatal, and nonfatal myocardial infarctions among the patients who received sildenafil and the other treatment groups.
Main analysis:The main analysis was conducted in patients who had at least one previous MI, had a history of stroke, had hypertension, or had previous MI or myocardial infarction within the last 12 months.
Results:The results of the study indicate that sildenafil and sildenafil at doses of 5 mg/day resulted in a higher cardiovascular event rate than the other treatment groups. The rate of fatal, nonfatal, and nonfatal myocardial infarctions was similar between the patients who received sildenafil and the other treatment groups. The rates of CHD death, CHD hospitalization, and use of other types of antihypertensive drugs among the sildenafil and the other treatment groups were similar between the patients who received sildenafil and the other treatment groups. Conclusions:Sildenafil and sildenafil at doses of 5 mg/day resulted in an enhanced cardiovascular event rate, compared with the other treatment groups. However, the rate of CHD death, CHD hospitalization, and use of other types of antihypertensive drugs among the sildenafil and the other treatment groups were similar.
